- Open the file from where you saved it locally on
your computer.
- When the macro/content alert pops up, click "Enable
Macro." Macro/Content must be enabled for the calculator to work.
- Click on the “Open Entry Form” button.
- Choose the subscriber's gender.
- Choose the subscriber's age.
- Choose “Yes” or “No” for Smoker.
- Choose a zip code prefix.
- Enter a Rate Level then click on the “Show Rates” button.
- Click on the “Clear Selections” button to start a new soft quote.
- Print as needed.
The approximate monthly rates for all plans (based on demographics entered) will be populated.
